Using the tab name was actually just added in the latest version! So yeah it could maybe use a bit more polish. How would you expect files to be named?

For the private tab bug, it happens on Firefox for Android's latest versions due to a bug in the app, you can't download anything when opening the add-on on private windows. You can still load the videos in private mode but to download them, open the add-on from a regular window. I have opened a bugzilla bug report about this problem. Otherwise sometimes when the add-on is not initialized yet nothing is detected. Usually opening the add-on first then loading your video fixes the problem.

I'm working on an update to make settings easier to browse. It will display a list of categories that you can drop down to reveal the associated settings (instead of having to scroll through and read everything)

The downloaded videos can be seen from the checkmark icon on the download button! I usually don't keep them separated if you want to try a download again, and so the order of the list stays consistent (from older to newer), but it's a good idea to have it as an option!

Which settings do you not understand? Maybe I can reword them!
